Online Signature Documents
If you have submitted an application for employment online, some additional forms are required in order to complete the process. The link provided below will redirect you to the digital forms suite. You will need the following information in order to access the required forms.
- Last name
- Last 4 digits of your Social Security Number
- PIN number provided by your recruiter
Fill in all required information, then click Lookup Forms. A list of forms will be displayed on the right side of the screen. Click on the provided links and complete each form. Be sure to click View Completed Form and then Save the form once each one has been finished.

Independent Contractor's Agreement
The Independent Contractor’s Agreement is your lease agreement with Quality Carriers. You will need to review and agree to the information outlined in this document when applying for an Owner Operator position. If you do not own a tractor, this document does not apply to you.
