Required Documents

The documents required to complete the hiring process vary depending on the position for which you are applying. The lists below will give you an idea of the information required in addition to your employment application.

Owner Operators

• Copy of CDL
• Copy of Social Security card or birth certificate
• DOT physical and drug screen
• 25-mile QC road test
• DOT tractor inspection
• Title or bill of sale (if tractor purchased within last six months)
• Tractor registration
• Copy of insurance certificates (Non-Trucking Liability and Occupational Accident/Workers’ Compensation. Quality Carriers offers a comparable insurance package available through NSM Insurance Group)

All Drivers

• Copy of CDL
• Copy Social Security card or birth certificate
• DOT physical and drug screen
• 25-mile QC road test
